A Needs Assessment is usually the first step towards accessing publicly funded residential aged care in New Zealand. It’s a free assessment carried out by a qualified health professional to understand your health, daily living needs and the level of support that will help you live safely and comfortably. The assessment looks at your individual circumstances and determines whether you would benefit from support at home or whether long-term residential care, such as Rest Home or Hospital Level Care, is the most appropriate option.How do I arrange a Needs Assessment?
There are several ways to arrange a Needs Assessment. You can speak with:- Your GP, who can refer you for an assessment.
- A hospital discharge team, if you’re currently in hospital.
- Your local Needs Assessment and Service Coordination (NASC) service, who coordinates assessments and support services within your region.
What happens during the assessment?
During the assessment, a qualified assessor will come to your home and will talk with you and, if you’d like, your family or support person about your health and everyday life. They’ll discuss things like:- Your general health and medical conditions.
- Your mobility and risk of falls.
- Personal care needs, such as showering and dressing.
- Memory or cognitive changes.
- How you’re managing at home.
- Support from family or carers.
- Your goals, preferences and concerns.
How long does the process take?
Timeframes vary depending on where you live and how urgent your situation is. If you’re being discharged from hospital or your needs are urgent, an assessment may be arranged quickly. In other situations, it may take a few weeks from referral to assessment.What happens after the assessment?
Following your assessment, you’ll receive a recommendation about the level of care that’s right for you. This may include:- Additional support to help you remain living independently at home.
- Short-term or respite care.
- Long-term residential aged care.

What happens next?
Once your Needs Assessment has been completed, your journey will usually look something like this: 1. Receive your assessment outcome Your assessor will recommend the most appropriate level of care and support. 2. Explore your care options If residential aged care is recommended, you can begin visiting care homes and finding the one that’s right for you. 3. Apply for funding (if eligible) If you meet the criteria, you can apply for the Residential Care Subsidy through Work and Income. 4. Complete your admission paperwork Your chosen care home will guide you through the remaining paperwork and help you prepare for moving in. 5.
